USMLE Step 1 guide

Who Can Sit USMLE Step 1: Eligibility and the IMG Route Through ECFMG and FSMB

Step 1 is open to students and graduates of LCME-accredited MD, COCA-accredited DO and ECFMG-eligible international schools. Since January 2026, international medical graduates register through FSMB after ECFMG acceptance. This guide sets out the order and the common blockers.

You can apply for Step 1 as a student or graduate of an LCME-accredited MD program, a COCA-accredited DO program, or a non-US medical school that meets ECFMG requirements. You must be in an eligible category both when you apply and on the exam day. International medical graduates (IMGs) first need their Application for ECFMG Certification accepted, then a MyIntealth account with identity verification, then an FSMB USMLE account whose name matches Intealth. US MD and DO students register through NBME. These rules took effect in January 2026, so older guides may be wrong. Confirm everything in the 2027 Bulletin of Information.

The two questions to answer first

Before you open any application, answer two questions. Which category are you in? And, if you are an IMG, which stage of ECFMG certification have you reached? The answers decide which website you use, what documents you need and how long the process takes. This guide follows that order.

Eligible categories

According to the 2027 Bulletin (page 8), Step 1 is open to:

  • Students and graduates of LCME-accredited MD programs in the United States and Canada. A caution applies to Canada: graduates of Canadian schools on or after 1 July 2025 are treated as IMGs under the transition rules.
  • Students and graduates of COCA-accredited DO programs in the United States.
  • Students and graduates of medical schools outside the US and Canada that meet ECFMG requirements, which is the IMG route.

You need to be in an eligible category on the day you apply and on the day you take the exam. This sounds simple, but it matters for students near graduation, for people who are between programs and for IMGs whose school listing or graduation status changes during the process. If your status might change, read the Bulletin section on eligibility and, if unsure, ask the registrar.

The change matters because many older guides say IMGs apply through ECFMG's system. That was true until the service transition. ECFMG still handles certification, which is the process of verifying your medical education credentials and your eligibility for US training, but it no longer takes the Step 1 application for new registrants.

The IMG pathway, step by step

  1. Check that your medical school qualifies. ECFMG eligibility depends on your school and your graduation status. Use the official ECFMG and Intealth information, not a forum, to confirm.
  2. Create a MyIntealth account. Intealth is the organization that runs the ECFMG identity and application platform. Complete identity verification in the way the site requires.
  3. Submit your Application for ECFMG Certification. The documents and medical school verification can take time. Start early, because the next step is blocked until this is accepted.
  4. Wait for acceptance. ECFMG must accept the application before a first Step application. Acceptance is the gate.
  5. Create your FSMB USMLE account. The name must match what Intealth holds. Differences in spelling, middle names, order or diacritics are common causes of delay, so fix them early.
  6. Apply for Step 1 and choose an eligibility period. The application fee was USD 695 for 2026 and 2027 when we checked on 25 October 2026. See the booking and fees guide.
  7. Receive your scheduling permit and book Prometric. Scheduling usually cannot be more than six months ahead.

Allow weeks, not days, for document verification. Medical school verification depends on third parties, and delays there cannot be compressed by effort on your side. Start the ECFMG application while you are still studying, not after your plan is complete.

What "eligibility period" means for you

When you apply, you select a three-month eligibility period, a window in which you must take the exam. The FSMB fee page and the Bulletin describe one contiguous extension for a fee. Because the window is short, align your preparation with it: your study plan ends before the period ends, with a buffer. Our study plan guide offers 8, 12 and 16-week schedules.

Common eligibility problems

  • Name mismatch. The name on your Intealth record, your FSMB account, your permit and your ID must match. Correct any difference before you apply.
  • Using outdated instructions. A guide that says IMGs apply for Step 1 through ECFMG describes the old process. Look for dates after January 2026.
  • Applying before ECFMG acceptance. It does not work, and waiting for acceptance wastes time if you started late.
  • Losing eligibility during the process. Graduation status and school recognition can matter. Check the Bulletin if your status could change.
  • Assuming Canadian rules are unchanged. Canadian graduates from 1 July 2025 follow the IMG route according to the transition FAQ.
  • Ignoring attempt limits. The limits apply to the Step, not to the application. You can take the same Step no more than three times in 12 months, and the Bulletin sets longer-term limits. See the retake guide.

Timeline planning for IMGs

The longest and least controllable part of the process is usually certification, not study. A realistic sequence runs as follows, and the lengths are not official figures, because they depend on your school and your documents.

What Step 1 gives you, and what it does not

Passing Step 1 is one required step. For IMGs, ECFMG certification involves more than the Steps, and the requirements are set by ECFMG and Intealth, not by the USMLE. We do not list those requirements here because we did not verify them, and they change. Read the current ECFMG information page for the full list. For residency applications, program-specific requirements apply as well.

After Step 1, the next exam is Step 2 CK, whose minimum passing score is 218 from 1 July 2025. Our guide to Step 1 versus Step 2 CK explains how the two exams differ in what they test and how they are reported.

Where to check, and what to save

  • 2027 Bulletin of Information. The authority for eligibility, scheduling, conduct and attempts.
  • USMLE Service Transition FAQs. The reference for who registers where and what changed in January 2026.
  • FSMB USMLE page and fee page. For IMG registration and fees.
  • NBME USMLE page. For US MD and DO students and graduates.
  • Save copies. Keep screenshots of your application, your permit and the fee page. If a rule or fee is disputed, you have a dated record.

Special situations to check before you apply

Some candidates do not fit the standard picture. The rules below come from the Bulletin and the transition FAQ, and each case needs a direct check against the current text.

  • You are close to graduation. Eligibility is judged at application and on test day. If your graduation date, your school's listing or your enrollment status changes in between, ask the registrar before paying.
  • You studied in more than one country. For IMGs, ECFMG decides what counts. Gather transcripts and diploma records early, since verification involves the schools.
  • You are a US citizen who studied abroad. You follow the IMG route, which means ECFMG and FSMB, not NBME. Citizenship does not change the pathway.
  • You changed your name. Align your records with Intealth first, then create or update the FSMB account. Do this before you apply, not after you receive a permit.
  • You sat Step 1 before the 2026 transition. Your prior attempts count toward the attempt rules. Keep dates and records for each attempt.

When two sources disagree, the 2027 Bulletin and the USMLE transition FAQ win over any third-party page, including this one. We cite them so you can check our reading.

How the identity chain fails in practice

Most delays come from the identity chain, not from the exam rules. A passport spells a name one way, a school diploma another, and an old Intealth profile a third. Each system compares them, so a single mismatch can hold up your permit. Before you apply, put the three records side by side and note every difference in order, spelling, punctuation and diacritics. If you find one, correct the record that is wrong and keep the confirmation. Doing this in week one of preparation costs an afternoon, while doing it after a permit is refused can cost weeks inside a short eligibility period.

Questions to ask yourself before you start

  1. Which category am I in today, and will it change before my test date?
  2. Do my names match across my passport, school records, Intealth and ID?
  3. Has ECFMG accepted my application, if I am an IMG?
  4. Does my eligibility period leave a buffer after my study plan ends?
  5. Have I read the 2027 Bulletin sections on eligibility, scheduling and attempts?

If you can answer yes to all five, you are ready to book. The Step 1 hub links to every guide, and the United States page lists the other exams on MedPass Exams.

Frequently asked questions

Who is eligible to take USMLE Step 1?

Students and graduates of LCME-accredited MD programs, COCA-accredited DO programs, and non-US medical schools that meet ECFMG requirements. You must be in an eligible category when you apply and on the day you take the exam, according to the 2027 Bulletin.

How do IMGs register for Step 1 in 2026?

Get your Application for ECFMG Certification accepted, create a MyIntealth account and complete identity verification, create an FSMB USMLE account with a name matching Intealth, then apply for Step 1. IMG registration moved from ECFMG to FSMB on 12 January 2026.

Do US MD and DO students register through ECFMG?

No. US MD and DO students and graduates register through NBME. ECFMG's role is certification for international medical graduates.

Are Canadian medical graduates treated as IMGs?

According to the USMLE Service Transition FAQ, Canadian graduates on or after 1 July 2025 are treated as IMGs. Check the FAQ for details that apply to your graduation date.

Can I apply for Step 1 before ECFMG accepts my application?

No. The Application for ECFMG Certification must be accepted before an international medical graduate can apply for a first Step. Start the certification application early, because document verification can take weeks.

What if my name differs between documents?

Fix it before you apply. The FSMB account name must match your Intealth record, and your ID on test day must match your registration. Name differences are a common cause of delays.

What does ECFMG certification require beyond Step 1?

The requirements are set by ECFMG and Intealth and can change, and we did not verify the current list for this page. Read the current ECFMG information before planning, since certification covers more than the exams.
