USMLE Step 1 guide

How to Register and Pay for USMLE Step 1 in 2026: FSMB, NBME and Prometric

The Step 1 application fee was USD 695 when we checked on 25 October 2026, and registration for international medical graduates moved to FSMB in January 2026. This guide shows the order of steps, what is confirmed, and what you must check at checkout.

Checked 25 October 2026: the Step 1 application fee is USD 695 for the 2026 and 2027 three-month eligibility periods, and costs stayed the same after the January 2026 registration transition. International medical graduates (IMGs) now apply through FSMB once ECFMG has accepted their certification application. US MD and DO students apply through NBME. After you apply you receive a scheduling permit, then book a Prometric appointment, usually no more than six months ahead. Extension, rescheduling, region-change and international test-center charges are not itemized on the pages we read, so confirm them at checkout.

The one fee we can confirm, and what we cannot

The FSMB fee page lists the Step 1 application fee as USD 695 for the 2026 and 2027 eligibility periods. The USMLE Service Transition FAQ says costs remain the same after the January 2026 move of registration. We read both pages on 25 October 2026. That is the only Step 1 price we state as fact.

The practical rule is simple. Read the fee schedule on the FSMB or NBME site on the day you pay, and screenshot it. Fees are dated, and 2028 and later periods may differ. If a blog or video tells you a number that is different, believe the registrar.

Where you apply: it depends on who you are

Registration changed in January 2026. This is the part of the process most outdated pages get wrong.

  • International medical graduates. Since 12 January 2026, IMGs register through FSMB. Before that, registration went through ECFMG.
  • US MD and DO students and graduates. They register through NBME. (Step 3 moved from FSMB to NBME on 26 January 2026, but this is outside the scope of Step 1.)
  • Canadian graduates. According to the transition FAQ, Canadian graduates on or after 1 July 2025 are treated as IMGs.

ECFMG still has a role. It handles certification, and it must accept your Application for ECFMG Certification before you can apply for a first Step. If you are an IMG, the order matters: ECFMG first, then your FSMB account, then the Step 1 application. The eligibility and IMG pathway guide covers the identity steps and the name-matching rule.

Booking steps, in order

  1. Confirm eligibility. You must be in an eligible category when you apply and on the day you test: a student or graduate of an LCME-accredited MD program, a COCA-accredited DO program, or an ECFMG-eligible international school.
  2. IMGs: set up your identity chain. Create a MyIntealth account, finish identity verification, and create an FSMB USMLE account. The name must match your Intealth record.
  3. Apply for Step 1 and select an eligibility period. The FSMB fee page lists three-month periods. Pick a window in which you are confident you can sit the exam, given your study plan.
  4. Pay the fee. The amount shown at checkout is binding. Keep the receipt.
  5. Receive your scheduling permit. You cannot schedule without it. The permit shows your eligibility period and any conditions.
  6. Schedule with Prometric. Use the permit details exactly. Scheduling usually cannot be made more than six months in advance. Choose a center you can reach without stress.
  7. Prepare ID. The name on your ID must match the name on your registration. A mismatch can prevent you from testing, so confirm the exact rules in the Bulletin.

Do not treat the six-month scheduling rule as a promise that a seat exists. Popular dates and some international regions fill early. Book soon after your permit arrives, then plan your study around the date.

The eligibility period is a deadline

The three-month eligibility period is the window in which you must take the exam. If it expires without a test, you will need to apply and pay again or buy the extension, so read the Bulletin rules on expiry. One contiguous extension is available for a fee, per the FSMB fee page and the 2027 Bulletin. Because the period is short, your study plan should work backward from the end date.

  • Apply late enough to be ready, early enough to secure a seat. A common approach is to apply when you are about 8 to 10 weeks from being ready, since seats and permits take time.
  • Keep a buffer inside the period. If your plan ends on the last day of the window, any illness or travel problem can force you into a paid extension or a new application.
  • Know your region rules. Changing the testing region may carry a charge and may need approval. Decide where you will test before you pay.

Budget the whole cost, not only the exam

The application fee is one part of a larger cost. The other parts depend on you, so we list categories and not invented prices.

  • Exam fee: USD 695 (confirmed, 25 October 2026).
  • Possible exam-related charges: extension, rescheduling, region change, international test-center fees. Check the schedule.
  • Study resources: question banks, books, courses. Our practice bank has its own price page, and we do not state prices on this guide.
  • Travel and accommodation: relevant if the nearest Prometric center is far.
  • ECFMG-related costs for IMGs: certification has its own fees and steps, which are separate from the Step 1 fee. Check the ECFMG and Intealth sites for the current list.
  • Retake reserve: if you do not pass, a retake needs another application fee. Plan for that possibility without expecting it.

Dates and timing that affect booking

These dates come from the USMLE transition FAQ and the 2027 Bulletin, checked 25 October 2026.

DateWhat changed or applies
26 January 2022Step 1 reporting became pass/fail only
1 July 2025Canadian graduates on or after this date are treated as IMGs
12 January 2026IMG registration moved from ECFMG to FSMB
26 January 2026Step 3 registration moved from FSMB to NBME
14 May 2026Step 1 changed to fourteen 30-minute blocks of up to 20 questions
2026 and 2027USD 695 application fee for the three-month eligibility periods

If a study guide says seven 60-minute blocks and a 45-minute break, it describes the exam before 14 May 2026. Look for the current wording in the Bulletin: fourteen 30-minute blocks, at least 55 minutes of break time and an optional five-minute tutorial.

Common booking problems and how to avoid them

  • Name mismatch. The name on your Intealth record, your FSMB or NBME account, your permit and your ID must agree. Fix any difference before you apply, not after.
  • Applying before ECFMG acceptance (IMGs). Your Application for ECFMG Certification must be accepted before the first Step application.
  • Using old instructions. A guide that says IMGs apply through ECFMG describes the system before 12 January 2026.
  • Late scheduling. Seats are limited. Schedule as soon as the permit arrives.
  • Letting the period lapse. Put the end date of your eligibility period in your calendar with two reminders.
  • Ignoring the retake rules. The attempt limits (no more than three attempts of the same Step in 12 months, and further limits beyond that) apply to the whole path. See the retake guide.

Before you pay: a short checklist

  1. Read the current Bulletin. The 2027 Bulletin of Information is the reference for eligibility, scheduling and conduct rules.
  2. Check the fee page on the day. Confirm USD 695 or the current figure for your period.
  3. Check your timeline. Your study plan, eligibility period and the six-month scheduling window should fit together. Our study plan guide helps with the plan.
  4. Check your identity documents. Valid ID, matching names, and (for IMGs) a completed Intealth verification.
  5. Know the exam format. Read the exam day guide so that the length of the day and break rules do not surprise you.

Worked timeline: from decision to test day

A calendar makes the rules concrete. The example below is for an IMG who already has an accepted ECFMG application and wants to sit about 12 weeks after starting to study. Dates are relative, and the lengths are planning guidance, not USMLE figures.

  • Week 0: confirm your FSMB account, your name match and your fee payment method. Read the current Bulletin sections on scheduling and conduct.
  • Week 1 to 2: start the study plan and apply when you can see the finish line. Applying too early starts the eligibility clock before you are ready.
  • Week 3: receive the permit and book Prometric. Choose a date inside your eligibility period with a buffer of at least two weeks after your plan ends.
  • Week 10 to 11: two full-length practice days. If results are unstable, you still have room to move the date within the rules, so read the rescheduling terms early.
  • Week 12: test day. Results follow in typically up to four weeks.

If your eligibility period is short and your plan is long, the order reverses: start studying before you apply, and apply only when you can book a date that you will realistically be ready for.

What we have not verified

Three questions we cannot answer from the pages we read are worth stating, because readers often ask them. We do not know the amount of the extension fee, the amount charged for rescheduling or changing testing region, or the cost of any specific international test center. We also did not verify refund conditions beyond what the Bulletin states generally. Treat any figure you see for these items as unconfirmed until you see it on the FSMB, NBME or Prometric site.

Frequently asked questions

How much does USMLE Step 1 cost in 2026?

The FSMB fee page lists a Step 1 application fee of USD 695 for the 2026 and 2027 eligibility periods (checked 25 October 2026). Other charges, such as extension, rescheduling, region change and some international test-center fees, are not itemized on the pages we read.

Who registers through FSMB and who through NBME?

Since 12 January 2026, international medical graduates register through FSMB. US MD and DO students and graduates register through NBME. ECFMG still handles certification and must accept your application before an IMG's first Step application.

How long is the Step 1 eligibility period?

The FSMB fee page lists three-month eligibility periods. One contiguous extension is available for a fee, according to the 2027 Bulletin. Work backward from the end date when you plan study time.

How far ahead can I schedule Step 1?

Scheduling usually cannot be made more than six months in advance. You need a scheduling permit before you can book a Prometric appointment, so apply when you are close enough to your planned test date.

Do I schedule Step 1 with Prometric?

Yes. Step 1 is delivered at Prometric test centers worldwide. After you receive your permit, you schedule through Prometric using the permit details exactly as issued.

What if my name does not match my ID?

Fix it before you apply. For IMGs, the FSMB account name must match your Intealth record, and your ID on test day must match your registration. A mismatch can stop you from testing.

Did the fee change when registration moved to FSMB?

No. The USMLE Service Transition FAQ states that costs remain the same after the January 2026 transition. Always confirm the figure on the FSMB or NBME site on the day you pay.
