The charge nurse is assigning four clients to an LPN/LVN who is working under RN supervision. Which client is most appropriate to assign to the LPN/LVN?
Answer: A. LPN/LVNs provide focused data collection and care for stable clients with predictable outcomes. Newly admitted, unstable or complex-teaching clients require the RN. Choosing the stable client keeps delegation within scope and safe. Based on: NCSBN/ANA National Guidelines for Nursing Delegation (2016); NCSBN 2026 NCLEX-RN Test Plan: Management of Care
A client has just been told by the provider about a new diagnosis of metastatic cancer. The client's adult children ask the nurse not to tell their parent the full extent of the disease. Later, the client asks the nurse, "What did the tests show?" Which response is best?
Answer: D. Autonomy and veracity require honest disclosure to a competent client who asks. The nurse does not give a diagnosis independently but ensures the provider promptly provides full information. Family requests do not override the client's right to know. Based on: ANA Code of Ethics for Nurses (2025) Provisions 1 and 2; NCSBN Test Plan: Management of Care
During a multiple-casualty incident, a nurse uses START triage in the emergency department parking area. An adult who cannot walk is breathing at 34 breaths per minute. Which triage category should the nurse assign?
Answer: A. In START, clients who can walk are minor. For others, absent breathing after airway repositioning is deceased or expectant, and a respiratory rate above 30/min is immediate. Perfusion (capillary refill or radial pulse) and mental status determine the rest. Based on: HHS/ASPR CHEMM START adult triage algorithm (Newport Beach Fire Department and Hoag Hospital); NCSBN 2026 NCLEX-RN Test Plan: Emergency response
