For medical graduates, the Saudi route is the SMLE: 200 single-best-answer questions with four options, in two blocks of 100 (120 minutes each) separated by a 30-minute break, delivered by Prometric in 11 testing windows a year. The pass mark is 560 on a 200-800 scaled score. The 2026 blueprint gives Medicine 30%, Obstetrics and Gynaecology 25%, Paediatrics 25% and Surgery 20%, with mastery levels from 1 to 3 on topics. You may sit up to four times a year, once per window. No separate SCFHS "GP exam" page appears on the pages we checked. Checked 21 October 2026.

Is the SMLE the "GP exam" for Saudi Arabia?

Saudi Arabia does not run a paper called a GP exam, as far as the SCFHS pages we checked show. The Saudi Commission for Health Specialties (SCFHS) classifies and licenses practitioners, and for medical graduates the published licensure exam is the Saudi Medical Licensure Examination (SMLE). Prep providers describe the SMLE as the Saudi general practitioner exam. A separate SCFHS GP page does not appear on the pages we checked, so confirm with SCFHS which exam your post and qualifications require. Experienced specialists, and applicants who do not fit the graduate route, may be classified through other SCFHS exams, so check the classification rules for your own case.

The blueprint

The SMLE is the only Gulf exam with a fully published weight table and topic list. The 2026 blueprint gives the following.

CategoryWeightStated variation
Medicine30%plus or minus 5
Obstetrics and Gynaecology25%plus or minus 5
Paediatrics25%plus or minus 5
Surgery20%plus or minus 5
Patient safety, preventive medicine and ethicsNo percentage statedListed as a fifth block

On 200 items those weights give about 60 medicine, 50 obstetrics and gynaecology, 50 paediatrics and 40 surgery items, before any variation. That is our arithmetic on the percentages and not a published item count. The blueprint allows up to 10% pilot questions, which is up to 20 of the 200 items.

The strong signal for revision is the pair of obstetrics and gynaecology and paediatrics: together they are half the paper. Our bank gives each 15%, so SMLE candidates should add hours there. See the obstetrics and gynaecology and paediatrics pages.

Mastery levels

The blueprint marks every topic with a mastery level from 1 to 3. Level 3 topics are those to master first. Examples named in the 2026 blueprint include coronary artery disease, heart failure, asthma, COPD, pulmonary embolism, peptic ulcer disease, inflammatory bowel disease, chronic viral hepatitis, sodium and potassium disorders, diabetes diagnosis and acute complications, iron deficiency anaemia, sickle cell disease, ischaemic stroke, pre-eclampsia, third-trimester bleeding, postpartum haemorrhage, ectopic pregnancy, gastroenteritis, febrile seizures, bronchiolitis, rickets, immunisation, appendicitis, bowel obstruction, burns and diabetic foot. Gulf-flavoured entries include brucellosis, MERS-CoV and COVID-19, rheumatic heart disease, travel health, sick leave, death certification and social media practice. The full list is in high-yield topics. Use the blueprint itself as your checklist: it is a primary document, free and dated 2026.

Format and pace

Each block is 100 items in 120 minutes, which is 72 seconds an item, followed by a 30-minute break and a second identical block. The format is four options, one best answer. The blueprint allows unscored pilot items, and you cannot tell which items they are, so answer all of them seriously.

A plan for the two blocks

  1. Block one, first pass. Answer at about 65 seconds an item. Flag anything that needs more than 90 seconds and move on.
  2. Block one, last 15 minutes. Return to flagged items. Leave no item blank.
  3. Break. Leave the room mentally as well: eat and drink, and do not review. The break is for recovery, and ruminating on block one costs block two.
  4. Block two, first pass. Start slightly slower to settle, then hold the same pace.
  5. Block two, last 15 minutes. Repeat the flag review.

Train for this structure with two timed 100-item blocks and a 30-minute break. The mid-exam fatigue dip is real, and practice is the only cure.

Scoring

The SMLE is reported on a scale from 200 to 800, and 560 is the pass mark. A scaled score is not a percentage of correct answers, and no published conversion appears on the pages we checked, so train to the blueprint, not to a percentage target. SCFHS licensure results are released 2 to 6 weeks after the testing window closes, with a feedback report. The Prometric SCFHS page also mentions 7 to 10 working days for some exams on its portal. Plan for the longer figure. See the scoring guide for how this compares with the other regulators.

Using the blueprint as a study map

Turn the blueprint into a tracker. List every topic under its category, tag it with its mastery level, and give it a pass count: three passes for level 3, two for level 2 and one for level 1. A pass means reading the guideline, doing at least ten questions and logging errors. Level 3 topics are common conditions in which a general practitioner is expected to act independently, so a wrong answer on them is the costliest. Level 1 topics are lighter; learn the headline facts and the safe default action, and do not spend a week on them.

The ethics and safety block

The fifth block carries no stated percentage, but its topics are rated and are among the easiest marks to win with a short, structured review. The 2026 blueprint lists the following.

Mastery levelTopics named in the blueprint
3Breaking bad news, patient privacy, confidentiality, conflict of interest, sick leave, research ethics, social media practice
2DNR and DNI, abuse (spouse, child, elderly), abortion, medico-legal issues and whistleblowing, impaired physicians
1Informed consent, discharge against medical advice, death certificate, fertility ethics, communication with peers' patients, second opinions

For abortion, DNR and end-of-life items, follow the local legal framework and keep to widely accepted principles. Article-level content of Saudi regulations is best checked at source, so avoid memorising article numbers. See the ethics, law and safety page.

Choosing a window

With 11 windows a year and up to four attempts, you can choose when to sit. Pick the window that follows your last stable mock by a week or two, not the first one you can reach. Book as far ahead as the three-month horizon allows so that you get your preferred centre, but only after you hold the scheduling permit and have checked its eligibility period. If you fail, you may not sit again in the same window, so the next window is your earliest retake. Use the gap to rebuild from the feedback report. A retake without analysis is unlikely to change the result. That is our reading of how the rule works, not SCFHS data.

Windows, permits and booking

  1. Submit the e-application to SCFHS.
  2. Receive a scheduling permit with an eligibility period.
  3. Book a Prometric slot in a testing window up to 3 months ahead. There are 11 windows a year.
  4. Bring the permit and an unexpired passport to the centre.
  5. Know the cancellation terms. Cancelling 16 to 30 days before costs a USD 50 fee. Cancelling 15 days or less before forfeits the exam fee.

The fee is not published on the pages we checked. It is shown at payment in the SCFHS portal. See booking and fees for the dated summary.

Attempts

You may sit up to four times a year, and only once per testing window. After you pass, two further attempts are allowed to improve your score. That is useful if you plan to use the score for something beyond licensure. The attempts rule is generous per year but the windows are discrete: a fail early in a window means waiting for the next one, so the plan should include time for the gap. The feedback report is the only structured diagnostic among the Gulf regulators that report scores, so use it.

Verification

DataFlow authenticates credentials for SCFHS. Its page lists paths for a new application, a report transfer and a status check, says a new SCFHS report can be issued within 5 days on a transfer if no additional documents are needed, and offers an applicant support centre. Other turnaround times and fees are not stated. See the DataFlow guide.

How the SMLE differs from the 150-item papers

  • Length. 200 items against 150, and two blocks against one.
  • Scoring. Scaled, not a percentage.
  • Information. A full blueprint and mastery levels, which the others lack.
  • Attempts. Four a year, tied to windows.

Use the study plan with the SMLE adjustments, and compare the others in the comparison guide. If you are weighing Saudi Arabia against another country, remember that our Saudi hub notes the SMLE score is also used in residency matching. If that applies to you, treat the first sitting seriously even though a retake is available, and confirm the use with SCFHS.

A 45-year-old man has had an anterior nosebleed for 20 minutes despite correct pressure. After he has been seated and a topical anaesthetic with vasoconstrictor applied, a single bleeding vessel is seen on the anterior septum. Which is the most appropriate next step?

A 68-year-old woman with type 2 diabetes and chronic kidney disease is admitted with fever, cough and hypoxia after her son, who works with camels, was admitted with pneumonia. Nasopharyngeal and lower respiratory tract samples are sent for MERS-CoV PCR and she is isolated. She is now on oxygen. Which statement about her treatment is correct?

A 64-year-old man with advanced lung cancer is breathless at rest. His respiratory rate is 22 breaths per minute and oxygen saturation is 96% on air. He is anxious but alert. Which is the most appropriate non-pharmacological measure to offer?
